Pediatric Care at Triangle Primary Care Associates
Your child’s health is our top priority—from newborns to teens. We provide expert pediatric care in a warm, family‑centered setting, making wellness approachable and comprehensive.
A True Medical Home for Kids
Following the American Academy of Pediatrics Medical Home model, we provide continuous, coordinated, and compassionate care. This approach improves vaccination rates, reduces hospital visits, and boosts satisfaction for children with or without special healthcare needs.
We get to know your family—that means anticipating developmental milestones, coordinating school and community support, and building trusting relationships with every paediatric visit.
Preventive Care, Always On Time
- Well‑child visits from infancy to adolescence, including screenings, immunizations, and growth monitoring.
- Developmental & behavioral screening, using tools like PSC or SDQ, to catch concerns early
- Nutrition & health guidance tailored to each stage of childhood.
Sick Visits You Can Depend On
Think of us as your first stop for minor injuries, fevers, colds, ear pain, stomach aches, and more. We also manage routine labs or imaging directly through the network, so fewer referrals and faster answers.
Integrated Behavioral Health for Kids & Teens
Behavioural health reflects childhood wellness. Our embedded team of psychologists and behavioural health consultants offers warm handoffs during visits, short-term interventions, and consults with psychiatrists when needed.
Families access care in familiar surroundings—with privacy, compassion, and results.
This integrated model has been shown to improve mental health, quality of life, and even school attendance—without raising overall costs
Chronic & Complex Care Coordination
Nearly half of children in primary care have at least one chronic condition. One-quarter manage multiple concerns like asthma, diabetes, ADHD, or neurological conditions—which often co-occur with behavioral health needs.
For our pediatric patients with chronic conditions, we offer:
- A medical home team, coordinating all your child’s providers, therapies, school communication, and community support.
- Proactive care coordination, home health liaison, and behavioral support to reduce family stress and improve outcomes.
How Pediatric Visits Work
Well Child Visit & Behavioral Screening
A typical well‑child visit includes growth checks, vaccinations, developmental and emotional-behavioral questionnaires. If concerns arise, a warm handoff connects you with our on‑site consultant immediately.
Sick Visit
Clearly describe symptoms. Your child may be seen promptly and may have same‑day access to a behavioral consultant if emotional or behavioral issues emerge during the visit.
Care Plan & Follow Up
We’ll create a plan—including referrals or behavioral sessions if needed—and follow up via portal, phone, or video as appropriate. Kids with special healthcare needs may get home-based support, school coordination, or care liaison services.
Meet Our Team
- Board‑Certified Pediatricians — experts in physical, emotional, and developmental health for all ages.
- Pediatric Nurse Practitioners — adept at well-child care, chronic management, and health education.
- Behavioral Health Consultants (PhD, LCSW) — provide brief, focused support during visits.

- Care Coordinators & Social Support Liaisons — connect your family to school, therapies, or home-based services.
Proven Benefits
- Behavior health integrated with well-child visits increases early intervention, whereas lack of such services leads to more needs.
- Pediatric medical home-type services further reduce ER visits, hospital stays, and total cost of care, all while improving parent satisfaction.
- Behavioral health integration with pediatric primary care increases provider efficiency-two provider visits per hour at least on those days-which means pediatricians see more kids and save time on each visit.
